My background is mostly Oil Refining & Petrochemical. I spent many years as a Turbomachinery Engineer and I have been in various other roles. I always used to tell our site people that I would “either fix it; or break it so well that they would get a new one.”
I enjoy instructing the Cause Mapping workshop and investiging actual incidents with both root cause analysis tools and Statistical methods. Summarizing with a picture is always a good plan.
